About the Role
This is a Field-Based role with a Home-Based office, offering the flexibility to telecommute while making a meaningful difference in the lives of Medicaid members in the San Marcos, TX market. As an HSS Clinical Coordinator RN - Level 1, you will play a critical role in assessing, planning, and implementing care strategies that are tailored to the unique needs of each patient.
Primary Responsibilities
- Care Strategy Development: Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
- Social Service Referrals: Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Care Plan Management: Manage the care plan throughout the continuum of care as a single point of contact, ensuring seamless transitions and high-quality care.
- Stakeholder Communication: Communicate with all stakeholders, including patients, families, healthcare providers, and community organizations, to ensure that health-related information is shared expeditiously and effectively.
- Patient Advocacy: Advocate for patients and families as needed, ensuring that their needs and choices are fully represented and supported by the healthcare team.

Requirements and Qualifications
To be successful in this role, you will need:
- Current RN License: A current, unrestricted RN license in the state of Texas.
- Community Health Experience: 2+ years of experience working in community health or a related healthcare role.
- Computer Proficiency: Intermediate level of computer proficiency, including Microsoft Outlook and Teams, and the ability to use multiple web applications.
- Reliable Transportation: Reliable transportation and the ability to travel to visit Medicaid members in their homes and/or other settings.
- Dedicated Workspace: Access to a designated quiet workspace in your home, with the ability to secure Protected Health Information (PHI).
Preferred Qualifications
- Long-Term Care Experience: 1+ year of experience with long-term care services and support, Medicaid, or Medicare.
- Microsoft Office Skills: Ability to create, edit, save, and send documents, spreadsheets, and emails.
- Knowledge of Integrated Settings: Knowledge of the principles of most integrated settings, including federal and State requirements.
- Commutable Distance: Reside within commutable distance of assigned duties.
