Job Summary
We're seeking a dedicated part-time Social Worker to join our Nursing Facility Assessment team. As a Social Worker with our team, you'll be responsible for reviewing Level II evaluations, collaborating with Nurses to develop determinations and specialized service recommendations, and creating guidance and training for Level II evaluators. This is a remote role, and we're looking for a highly skilled and experienced professional to join our team immediately.
Key Responsibilities
- Review and Analysis: Collaborate with Nurses to review Level II evaluations for individuals with MI or ID/DD, and develop determinations and specialized service recommendations.
- Guidance and Training: Create guidance and training materials for Level II evaluators regarding specialized services, ensuring that they have the necessary knowledge and skills to provide high-quality care.
- Timely Assistance: Provide timely assistance to Level I and Level II evaluators, referring cases to appropriate staff and community services as needed.
- Documentation: Document services provided using established tracking strategies, ensuring that accurate and comprehensive records are maintained.
- Collaboration and Communication: Work closely with Nurses, evaluators, and other stakeholders to ensure that individuals receive the support and services they need.
Essential Qualifications
- Education: Master of Social Work (MSW) from an accredited college or university.
- Licensure: Licensed to practice in the State of Kansas.
- Experience: Minimum of two years' experience in nursing facilities, mental health centers, or developmental disability services.
- Skills: Strong written and oral communication skills, organizational abilities, and interpersonal skills.
- Computer Skills: Intermediate to advanced computer skills, especially in Excel.
Preferred Qualifications
- Experience with MI or ID/DD populations: Experience working with individuals with Mental Illness or Intellectual/Developmental Disabilities.
- Leadership or supervisory experience: Experience in a leadership or supervisory role, with a focus on staff development and training.
- Familiarity with regulatory requirements: Familiarity with regulatory requirements and standards related to nursing facilities, mental health centers, or developmental disability services.
