As a Patient Navigator, you are responsible for collaborating with other health care, mental health, social service, and community providers to assist patients in accessing additional services and supports that will help patients thrive in recovery and in health. Must work collaboratively with staff exhibiting flexibility and a positive attitude. Patient service is a key priority with the goal of fostering an environment which promotes patient comfort and trust. For OHH members receiving coordinated case management services, you will be responsible for care coordination, health promotion, individual and family support services and referral to community and social support services.
DUTIES AND RESPONSIBILITIES
Always maintain professionalism with appearance and conduct at all times
Adhere to policy and procedures during all activities
Perform all duties within HIPAA regulations and always maintain patient confidentiality
Provide linkage and care coordination around secondary health conditions
Provide case management services for patients needing housing, employment, and other services to achieve success in recovery and life
Utilize our EMR to document clients’ progress
Arrange Logisticare rides for patients as needed
Do clinical screens as needed
Enter care coordination notes for OHH members arriving for MAT counsel appointments into EMR and OHH billing spread sheet

EDUCATION AND EXPERIENCE
Hold an MHRT-C certification in the state of Maine AND
Have a minimum one year of experience in a SUD or medical setting
High comfort level with technology, including EMR, email, and software and mobile apps (e.g., Google Apps)
