Join Our Team as a Home Health Aide

We are seeking a compassionate and skilled Home Health Aide to join our team in Placerville, CA. As a Home Health Aide, you will play a vital role in coordinating and providing care to our hospice patients, addressing their personal care needs, and promoting their comfort and well-being. Key Responsibilities:

  • Coordinate and provide care to hospice patients, including personal care, emotional support, and comfort measures
  • Calm, soothe, and empathize with patients and families, addressing their strong emotions and concerns
  • Participate in interdisciplinary team meetings, collaborating with healthcare professionals to deliver superior patient care
  • Assist with observing patients, documenting care, and reporting any changes or concerns to the healthcare team
  • Collaborate on cases, participate in training and development, and engage in other organizational activities as required

Your Experience and Education

To be successful in this role, you will need:

  • A Certified Nursing Assistant Certificate issued by the State Department of Public Health (required)
  • A Certified Home Health Aide certification issued by the State Department of Public Health (preferred)
  • Current BLS/CPR certification
  • At least one year of aide experience, with one year of hospice experience preferred
  • Possession of a valid California Driver's License, access to a reliable automobile, and minimum car insurance coverage
  • Excellent observation, problem-solving, verbal, and written communication skills
  • Demonstrated ability to efficiently use a computer, with prior electronic medical record (EMR) experience preferred
