As a Case Manager RN, you'll play a critical role in coordinating care for our members, ensuring they receive the best possible outcomes and experiences.

Job Summary

We're seeking an experienced Full Stack Case Manager RN to join our team in a 100% remote position. As a Case Manager RN, you'll be responsible for assessing the health status and care coordination needs of our members, developing and implementing individualized case management plans, and advocating for patients to the full extent of existing health care coverage. You'll work closely with our members, healthcare providers, and other stakeholders to ensure seamless care coordination and optimal outcomes.

Key Responsibilities

As a Case Manager RN, your key responsibilities will include:
  • Applying data-driven methods to identify members who require individualized case management programs and/or referrals to alternative healthcare programs.
  • Conducting comprehensive clinical assessments to evaluate the needs of our members and develop flexible approaches based on their needs, benefit plans, or external programs/services.
  • Advocating for patients to the full extent of existing health care coverage, promoting quality, cost-effective outcomes, and making suggestions to improve program/operational efficiency.
  • Identifying and escalating quality of care issues through established channels.
  • Providing an exceptional level of customer service to our members and stakeholders.
  • Utilizing assessment techniques to determine member's level of health literacy, technology capabilities, and/or readiness to change.
  • Utilizing influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum levels of health.
  • Providing coaching, information, education, and support to empower members to make ongoing independent medical and/or healthy lifestyle choices.

Requirements

To be successful in this role, you'll need:
  • An active, current, and unrestricted RN license in the state of residence.
  • 3+ years of acute care experience as an RN, with a willingness and ability to obtain additional state licenses upon hire (paid for by the company).
  • Strong decision-making skills, well-developed interpersonal skills, and the ability to exercise independent and sound judgment.
  • Excellent organizational and time management skills, with the ability to manage multiple priorities and demonstrate strong teamwork skills.
  • A compact RN license or multi-state licensure, Certified Case Manager (CCM), Managed Care experience, Hospital experience in ICU, CCU, Step Down, ER, or OR, and Telephonic case management and/or telephonic experience are preferred qualifications.

Education

We require an Associate Degree, with a Bachelor's degree preferred.

Compensation and Benefits

We offer a competitive salary range of $54,095.60 - $116,760.80, with a bonus, commission, or short-term incentive program in addition to the base pay range. Our comprehensive benefits package includes:
