Responsibilities

?? Communicate with stakeholders to ensure quality coordinated care and services are provided promptly.

?? Advocate for members and families to ensure their needs and choices are represented and supported by the healthcare team.

?? Visit Medicaid members in their homes and other settings, including community centers and hospitals.
Requirements
?? Current, unrestricted RN license in the state of Texas.

?? 2+ years of experience working within the community health setting or in a healthcare role.

?? Intermediate level of computer proficiency, including Microsoft Outlook and Teams.

?? Reliable transportation and ability to travel within the assigned region.

?? Access to a designated quiet workspace at home with the ability to secure Protected Health Information (PHI).

?? Reside in a location with access to a UnitedHealth Group approved high-speed internet connection.
Nice-to-haves
?? 1+ years of experience with long-term care services and support, Medicaid or Medicare.
