The Care Navigator - Behavioral Health position at Sunshine State Health Plan is a vital role aimed at transforming the health of our communities by providing quality, cost-effective healthcare outcomes for our members. The individual in this position will develop, assess, and coordinate care management activities tailored to the unique needs of each member. This involves creating personalized care plans or service plans and educating members and their families or caregivers about the available services and benefits that can enhance their healthcare access and quality. In this role, the Care Navigator will evaluate the needs of members, identify barriers to care, and recommend resources to facilitate the best possible outcomes. This includes working closely with members to develop ongoing care plans, identifying necessary providers, specialists, and community resources. The Care Navigator will also provide psychosocial support and resource assistance to members and caregivers, helping them access local services related to employment, education, housing, and more. Coordination is key in this position, as the Care Navigator will liaise between members, their families, and the care provider team to ensure timely access to identified services. Monitoring progress towards care plan goals and collaborating with healthcare providers for necessary revisions will also be part of the responsibilities. The role may involve on-site visits to assess member needs and provide education on healthcare options and benefits. Overall, the Care Navigator plays a crucial role in advocating for members and ensuring they receive the high-quality care they deserve.
Responsibilities

Requirements
? Bachelor's degree in a related field.

? 2 - 4 years of related experience in healthcare or care management.

? Graduate from an Accredited School of Nursing if holding clinical licensure.

? Current state's clinical license preferred.
