The Care Coordinator manages, updates, and oversees all services and service plans necessary for resident care through service plans.

Responsibilities
? Compiles health information and assesses biometric markers to detect deficiencies in care, providing disease-specific education and interventions tailored to the identified gaps with direction and oversight from a provider, qualified healthcare professional, or registered nurse.
? Communicate and elucidate policies and protocols to staff, patients, guests, and government agencies as needed.

Qualifications
? Gaining a high school diploma or GED signifies the culmination of academic requirements at the secondary level
? CPR certified
? CNA/HCA certified
? LPN Preferred
? Candidates with one year of experience in a long-term facility are preferred
