Care Management Associate (Remote)

Responsibilities:
As a Care Management Associate, you will be a key partner in helping individuals navigate their healthcare needs. Your responsibilities will include:
- Managing a high volume of inbound calls (40-60 per day) to assist members with their healthcare inquiries.
- Conducting initial assessments and coordinating care group assignments.
- Utilizing established protocols and systems to identify member needs, make appropriate referrals, and ensure access to necessary healthcare services.
- Monitoring member cases for potential issues and escalating concerns to clinical staff.
- Building and maintaining accurate member records within our systems.
- Providing support to colleagues by documenting calls, taking messages, and resolving issues.
- Adhering to compliance requirements and maintaining confidentiality.
- Participating in team meetings and contributing to process improvements.
Qualifications:
- Minimum of 6 months of experience in a high-volume call center environment is required.
- Proficiency with computers, including Microsoft Word, Outlook, and Excel.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to multitask, prioritize, and work independently.
- Strong empathy and a commitment to providing exceptional customer service.
- High school diploma or equivalent required.
Work Schedule:
Standard working hours are Monday through Friday, 12:30 pm - 9:00 pm EST. Occasional Saturday shifts and special occasion shifts may be required.
Benefits:
We offer a competitive salary of $24-$30 per hour and a supportive remote work environment.
