Job Overview

We are seeking an experienced and compassionate Licensed Masters Social Worker to join our team in a remote, full-time capacity. As a Social Worker at blithequark, you will play a vital role in partnering with registered nurse case managers to provide comprehensive support to our members. Your expertise will be instrumental in assessing, planning, and advocating for members with complex needs, including financial, transportation, safety, and housing challenges. Through telephonic and electronic interactions, you will empower members to make informed lifestyle and medical choices, while collaborating with cross-functional teams to connect them with valuable community resources.

Key Responsibilities

  • Conduct thorough assessments to identify members' needs and develop personalized plans to address them
  • Research and provide information on available resources, including financial assistance, transportation, safety, and housing options
  • Collaborate with registered nurse case managers and other clinical staff to deliver comprehensive support and care coordination
  • Empower members to make positive lifestyle and medical choices through education, guidance, and support
  • Utilize electronic health records and other systems to document member interactions and track progress
  • Participate in ongoing training and professional development to enhance skills and knowledge in medical social work and case management

Essential Qualifications

  • Active and unrestricted LSW, LMSW, LCSW, or LISW license in the state of residence
  • Master's Degree in Social Work (MSW) from an accredited institution
  • Minimum 2 years of experience in medical social work or case management, with a strong understanding of community resources and social determinants of health
  • Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a remote environment
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities

Preferred Qualifications

  • Experience working with diverse populations, including vulnerable or underserved groups
  • Knowledge of electronic health records and other healthcare technology systems
  • Certification in case management or a related field, such as CCM or ACM
  • Bilingual or multilingual skills, with the ability to communicate effectively with members who speak languages other than English

Compensation and Benefits

The typical pay range for this role is $46,597.20 - $101,228.40 per year, depending on experience, education, and location. In addition to your base salary, you will be eligible for a range of benefits, including medical, dental, and vision coverage, a 401(k) retirement savings plan, and an Employee Stock Purchase Plan.
