Responsibilities
? Collects healthcare information and examines biometric data to recognize deficiencies in care, delivering disease-specific education and interventions specific to the identified gaps under the guidance of a provider, qualified healthcare professional, or registered nurse.
? Communicate policies and protocols to staff, patients, visitors, and government officials as directed.

Qualifications
? Successfully completing high school or earning a GED are significant milestones that demonstrate educational proficiency
? CPR certified
? CNA/HCA certified
? LPN Preferred
? Preference will be given to those with one year of experience in a long-term care facility
