The Care Coordinator handles the coordination, monitoring, and updating of services and service plans that cater to resident care needs.

Responsibilities
? Gathers health data and reviews biometric measurements to pinpoint gaps in healthcare and provide disease-specific education and interventions designed for the identified deficiencies under the direction and supervision of a provider, qualified healthcare professional, or registered nurse.
? Describe procedures and rules to employees, individuals, visitors, and authorities as appropriate.

Qualifications
? Obtaining a high school diploma or a General Educational Development (GED) credential are both important educational achievements for young adults
? CPR certified
? CNA/HCA certified
? LPN Preferred
? One year of experience in a long-term care facility is the preferred qualification
