Job Description:
The successful candidate will be responsible for:
- Leading and supervising a team of care management staff, providing guidance, support, and performance feedback
- Overseeing caseloads, ensuring timely communication with members, providers, and community-based organizations
- Providing training, resolving staff issues, and identifying areas for program improvement
- Collaborating with management to perform annual reviews, disciplinary actions, and goal setting
- Establishing and sustaining relationships with community systems to facilitate linkage, referral, advocacy, and follow-up
- Assisting with member concerns and grievances, and participating in internal and external audits
Skills and Qualifications:
To be successful in this role, you will need:
- Excellent communication, negotiation, and interpersonal skills
- Ability to work effectively in a cross-functional team environment
- Strong computer literacy and skills
- Self-motivation to seek knowledge and improve personal and professional performance
- Regular attendance and a strong work ethic
Preferred qualifications:
- Master's degree
- Spanish language proficiency (strongly preferred)

Work Schedule:
